Chronic disease management e-learning package is an online learning program that consists of eight disease specific modules that cover the topic areas of arthritis, asthma, cancer, coronary heart disease, dementia, diabetes and chronic kidney disease. The package content has been developed by expert organisations ensuring best practice is contained in each module and provides participants with an introductory level knowledge and information to support the monitoring and support role of patients.

The package is a flexible, self-paced learning tool that enables individuals to design their own study timetable. A certificate of completion is awarded upon successful completion of each module and up to 30 CPD hours are available for nurses upon successful completion of the package for continuing professional development.
